This community partnership, modeled on Seniors Count of Manchester, is unique in that it is a community-based prototype allowing for program leadership and growth to come through community leaders and service providers.
Rockingham Community Action (RCA) serves as the agency sponsor. The group also includes leadership representatives from Service Link, Living Innovations, Portsmouth Regional Hospital, Seacoast Media Group, NH Bureau of Elderly and Adult Services, service providers, and senior advocates. The partnership’s mission is to reach out and help older people in our community who are isolated, frail, or at-risk. The Community Liaison assists with housing, medical transportation, or financial problems that threaten the senior’s health or independence.
In 2012, the liaison served 56 seniors and their families, providing ongoing support for non-medical needs to seniors who do not have access to services through traditional medical or social service channels. In addition, $5,000 was accessed through the Flex Fund for minor car and home repairs and safety supports to help seniors remain safely in their own homes. Improving the way our community cares for frail older adults ultimately creates a better life for all.
